What is telephone health coaching?
Telephone health coaching provides people with support from a health professional to make healthy changes.
It focuses on what the client can actively do to make goals, and reach the goals they set for themselves, to achieve positive health outcomes. Health coaches provide effective motivation, tools and techniques to clients to assist them to make permanent changes in their life.
The telephone health coach will work with the client to devise appropriate health goals and a plan of action. They then provide follow up sessions to keep the client focused and on target for success.
Coaching consists of up to five telephone consultations. The first session will last up to 45 minutes, and the subsequent sessions for between 15 to 20 minutes.
Who are the health coaches?
All of the health coaches are health professionals trained in the Health Coaching Australia model of Health Change. The HCA Model of Health Change introduces evidence-based change protocols in a structured 10 step format that makes it easy for health professionals to apply.
Who can use health coaching services?
Telephone health coaching is ideal for people who are ready to make positive, long lasting health changes but want the additional support from a health professional. They may have work or family commitments or live in a rural location.
How long does it take?
Telephone health coaching usually involves a course of five personal sessions with a health professional. The first session will last up to 45 minutes, and subsequent sessions will last for 15 to 20 minutes.

Life! HELPING YOU PREVENT DIABETES, HEART DISEASE & STROKE is a type 2 diabetes prevention program funded by the Victorian Government and coordinated by Diabetes Australia – Vic.
People at risk of type 2 diabetes who are not able to attend a Life! course in person are eligible for telephone health coaching at no cost.
